New Patient Registration

New patients are requested to complete a registration form together with health and ethnicity questionnaires, prior to seeing a doctor for a new patient health check, if required.

Patients are registered with a named GP who works with a buddy GP.  When making an appointment you may express a preference as to which doctor you would like to see.

You will be asked to provide ID and every adult must attend in person to register.  In order to register with this Practice you must be within our practice boundary, please contact reception to confirm your address details. 

It is at the discretion of each doctor to accept you on to the list.  If refused, you would be notified within 14 days of the reason for this decision (which would not be on grounds of disability, medical condition, age, appearance, race gender, social class, religion or sexual orientation) and given information on how to find a new practice.
